Office Policies and Insurances
We are providers for DMBA, PEHP, Blue Cross, Delta Premire, United Concordia, and Utah Local Government. We also accept most other insurances. The majority of insurance companies allow the patient to go outside of their network at a slightly reduced reimbursement. We will work with you and your insurance to provide the best care for the most affordable cost.

If you do not have insurance we offer a 10% discount if you pay at time of service.

Referrals
We appreciate any referrals to our practice. We credit $75 to your account if you refer a new patient to our office. Call our office for details.
